You also release Fountain of Youth Aesthetics & Wellness and its providers from liability related to known risks and potential complications associated with treatment
Pregnancy Waiver
Certain wellness treatments and therapies may not be recommended during pregnancy or while breastfeeding. Please inform your provider if you are pregnant, nursing, or believe you may be pregnant so we can ensure the safest treatment options for you and your baby
